Delayed 4th Degree Repair

Introduction: Missed 4th degree perineal trauma is a main causal factor for rectovaginal fistula. Many a times these cases were referred to colorectal for secondary repair. Frequently colostomy is being performed leading to displeasure among the patients.

Case Presentation: This is a case report of a 62 year old lady with rectovaginal fistula. She sustained a 4th degree tear more than 20 years ago. She complained of faeces in vagina for 20 years. This video showed rectovaginal fistula being repaired vaginally. She did not need a colostomy. She recovered well, there no more faecal matter in vagina. There was no faecal or flatus incontinence.

Conclusion: It is important to identify 4th degree tear and repair it post partum. Rectovaginal fistula can repair vaginally successfully without needing a colostomy.
