The incidence of petroclival meningioma in pregnant women is rare. A 36-year-old gravida 5 para 2+2 in her 27 weeks of pregnancy presented with recurrent falls, left sided body weakness associated with slurred speech for one month.  She was diagnosed antenatal by MRI and she was able to carry the pregnancy up to 36 weeks. We performed a successful caesarean delivery under general anaesthesia at 36 weeks. CECT brain, noted enhancing extra axial mass in the left posterior fossa extension to the left temporal lobe measuring 5.5×6.3×5.2 cm with perilesional oedema. The mass causes compression to the brainstem, basal cistern and fourth ventricle. After 5 months post-delivery patient underwent left rectosigmoid suboccipital craniotomy and tumour excision. HPE report came back as meningothelial meningioma. Patient’s condition is still the same, poor prognosis and it is unlikely to return to premorbid condition.
