Laparoscopic Reversal of Tubal Sterilisation with Aid of Feeding Tube

Tubal sterilization is a method of permanent contraception. In order to meet the demands of patients who regret their tubal sterilization and a small but increasing number of women request reversal of sterilization, laparoscopic reversal of tubal sterilization had been developed.

This discussion reappraises the role of such a technique. Tubal reanastomosis can be divided into 6 types according to the segment involved: intramural-isthmic, intramural-ampullary, isthmic-isthmic, isthmic-ampullary, ampullary-ampullary and ampullary-infundibular. Technically the easiest ones to reverse are those in which the proximal and distal lumen was of relatively equal size. The principal technique of laparoscopic reversal of tubal sterilization was end to end anastomosis. The damaged segment was excised, conserving as much healthy fallopian tube as possible. The patency of the fallopian tube was confirmed by passing a feeding tube through the fimbrial end towards to proximal end. This acts as a guide for the surgeon in order to visualize the lumen clearly. The reversal of tubal sterilization by laparoscopy involves gentle handling of tissue, meticulous haemostasis, the use of delicate instruments and fine non-absorbable sutures, careful dissection, accurate tissue approximation and magnification techniques.

Until more valid statistical data are collected, it can be stated that laparoscopic reversal offers a potential increase in the intrauterine pregnancy rate when used to reverse tubal sterilization. Therefore it is important for training and to be experienced in laparoscopic reversal of tubal sterilization in order to restore the normal tubo-ovarian function and improve pregnancy rate.
