Ruptured Pulmonary Arteriovenous Malformation during Pregnancy

Pulmonary arteriovenous malformations (PAVMs) are abnormal communications between pulmonary arteries and veins. It is rare in the general population. Physiological changes during pregnancy can worsen PAVM and increase its risk of haemorrhage. This case report describes a case of haemothorax as a result of ruptured PAVM and discuss its management during pregnancy.

A 33-year-old primigravida at 25 weeks of gestation presented with acute onset of shortness of breath due to a large haemothorax. Computed tomography angiography confirmed the presence of PAVM and she underwent successful embolization. She subsequently carried her pregnancy to term and delivered via assisted vaginal delivery.
